Select a report from the list below and click on the corresponding "Preview/Request Report" link for that report.  From that page you can submit a request to have a Scientific Collectors Permit Report generated and an MS Excel 2003-format (.xls) spreadsheet will be sent to the e-mail address for your FWIS account (see "OPTIONS" in the top right of any FWIS page).

Please check over the preview version of the report carefully and if you notice errors in that output, you can correct them using the "Add/Edit Data" / "Stream and Site Codes" or "Sample Events" navigation links.

The Ontario government issues multi-year permits and the Preview/Request Report link limits results to a given year. You can remove that restriction by editing the URL for the report and removing the "&SCYear=nnnn" option from the URL.

You can also edit the URL to add or change a restriction on which streams are included in the report. Normally all sampling for a given year includes sites on all streams that match the permit number. To constrain the report to sites on one or more stream codes, add the following to the report URL: "&SCArr=sc1,sc2,...scn" where sc1, sc2, etc. are the desired stream codes.
